The Santa Clara County Community Benefit Coalition conducts a triennial Community Health Needs Assessment (CHNA) to identify and document critical health needs in our community. These needs inform El Camino Healthcare District Community Benefit Program’s strategies to improve the health and well-being of individuals who live, work or go to school in our District.
Our Community Benefit Advisory Council, comprised of board liaisons, physicians and representatives from the community, prioritized and selected the health needs identified by the 2016 CHNA. Selection criteria included the number of community members affected, the impact on the community, and whether the District has the expertise and resources to make an impact.
To guide community benefit grant funding in fiscal years 2017-2019, the Council identified 12 health needs from the CHNA, falling under three health priority areas.
Healthy Body: Grants support efforts to prevent the onset of disease and provide enhanced access to primary care, chronic disease management and oral health.
Healthy Mind: Grants promote access to services that address mental health, depression, and anxiety-related issues, as well as dementia, domestic violence, and substance use for youth and adults.
Healthy Community: Grants support efforts to improve the overall health of the community through increased access to transportation, falls prevention, community screenings and health education.
These three health priority areas guide the District’s activities and grant funding, allowing us to stay true to our purpose: To provide a range of healthcare services that will foster good physical and mental health in the communities we serve.
